Whether you’ve just been diagnosed with prediabetes or you’ve lived with Type 2 diabetes for years, lifestyle medicine can help. The American College of Lifestyle Medicine (ACLM) has studied healthy lifestyle habits and found they can be as effective as medication for treating diabetes.

Diabetes care at Kinwell Primary Care

Kinwell Primary Care offers a one-session introduction and weekly program for patients with prediabetes and diabetes on adopting healthy lifestyle practices. Participants will learn about sustainable lifestyle changes that can improve their health, lower their blood sugar, and help them make lasting changes to their overall well-being, such as adjusting their diet and increasing movement (doesn’t need to be exercise).

Lifestyle medicine classes about prediabetes, diabetes, and hypertension

Lifestyle Medicine: Intro to Diabetes is a weekly, virtual group program that helps patients take control of their diabetes using the ACLM’s 6 pillars of health and well-being. With more than 30 years of clinical experience, Dr. Kathleen Tonti-Horne, Kinwell’s director of lifestyle medicine, will teach patients how to build healthy habits that can help them manage their diabetes and possibly reduce their need for medication. This 1-hour session explores lifestyle medicine as a tool for making sustainable changes and achieving health goals. The final 20 minutes are reserved for questions and individual concerns.

Presenter: Kathleen Tonti-Horne, MD

Dr. Kathleen Tonti-Horne earned her Bachelor of Science degree from Dominican College of San Rafael and her Doctor of Medicine degree from the University of California, Irvine. She completed her residency in Family Practice at the Eau Claire Family Medicine Clinic at the University of Wisconsin and is certified by the American Board of Family Practice.

With more than 30 years of experience practicing medicine in California, Wisconsin, and Washington, Dr. Tonti-Horne brings a passion for improving health outcomes through accessible, evidence-based care. Prior to joining Kinwell, she served as Medical Director at the VAMC community-based outpatient clinic in Silverdale and previously as Medical Director of Monterey County Jail, where she focused on improving health outcomes for underserved populations. As a physician for Kinwell, she has been instrumental in creating group programs for the treatment of chronic conditions through lifestyle changes.
