Ongoing support in Lifestyle Medicine

Patients who complete a Lifestyle Medicine program may continue their exploration of healthy living through our Continuing the Journey program. These monthly group appointments explore the six pillars of good health as defined by the American College of Lifestyle Medicine: nutrition, physical activity, restorative sleep, social connection, stress management, and the avoidance of risky behaviors and substances.

These appointments are held via secure MyChart Zoom session once a month on Thursday at 11:45 am and 5:15 pm. Patients can pick and choose what sessions they wish to attend.

Monthly-Thursdays 11:45 – 1:00 pm AND 5:30 – 6:45 pm, 2026.
