She will shed light on how simple, sustainable lifestyle changes can have a measurable impact on diabetes prevention, management, and even remission.

Dr. Tonti-Horne received her Doctor of Medicine degree from the University of California, Irvine, and has practiced in California and Washington for over thirty years. As medical lead of lifestyle medicine for Kinwell, she has been instrumental in creating group programs for the treatment of chronic conditions through lifestyle changes.

“Managing diabetes isn’t just about medication, it’s about empowering yourself with the right habits and knowledge,” said Dr. Tonti-Horne. “Lifestyle medicine gives people the tools to take charge of their health in meaningful ways.”

Dr. Kathleen Tonti-Horne earned her Bachelor of Science degree from Dominican College of San Rafael and her Doctor of Medicine degree from the University of California, Irvine. She completed her residency in Family Practice at the Eau Claire Family Medicine Clinic at the University of Wisconsin and is certified by the American Board of Family Practice.

With more than 30 years of experience practicing medicine in California, Wisconsin, and Washington, Dr. Tonti-Horne brings a passion for improving health outcomes through accessible, evidence-based care. Prior to joining Kinwell, she served as Medical Director at the VAMC community-based outpatient clinic in Silverdale and previously as Medical Director of Monterey County Jail, where she focused on improving health outcomes for underserved populations. As a physician for Kinwell, she has been instrumental in creating group programs for the treatment of chronic conditions through lifestyle changes.
